How do I register a birth in Ontario?
- After you give birth, the hospital or midwife will give you
a form called a Statement of Live Birth (Form 2). This is your
child’s permanent identity record. Fill out the form, ensuring
the information is clear and accurate.
- Completion of Form 2, Statement of Live Birth, is required
within 30 days of the birth and is to be signed by both parents.
It is important to mark
Box 20a indicating that both parents agree or do not agree with the last
name chosen
for the child.
- Form 2, Statement of Live Birth, may be completed
and signed by the mother only if the father is not being named
on the registration.
If this is the
case, put “information not provided” on line 6.
- If both parents
are incapable, the statement may be completed and signed by another
person acting on the parent's behalf, provided a statutory declaration
is submitted. Please ask for further information at the Clerk's
Office.
- Form 2, Statement of Live Birth, is a permanent legal
record and must be completed accurately. Please refer to the
instructions
attached to Form
2
- Mail or deliver the Form 2, Statement of Live Birth, to the
Clerk's Office at the Municipal Office, 330 Wallace Avenue North,
Listowel ON N4W 1L3 along with the applicable administrative
fee in the
envelope
provided.
- In accordance with Bill 26 and amendments to the Municipal
Act, municipalities have the authority to impose user fees for
services.
An administrative
fee in the amount of $25.00 is being charged by the Municipality of North Perth for the registration of a birth. Please
enclose payment in the amount
of $25.00 (make cheques payable to the Municipality of North Perth) along
with
your Form
2, Statement
of Live Birth. Alternatively, you may pay in cash by attending
in person at the
Clerk's Office at the Municipal Office, 330 Wallace Avenue North,
Listowel ON N4W 1L3. Payment of this fee does not represent payment
towards a birth certificate.
- Once the birth has been registered you will
receive confirmation of same from the Office of the Registrar
General in Thunder Bay.
This acknowledgement
does not replace a birth certificate.
Once your baby's birth is registered, you can apply for a birth
certificate
Birth
Certificates can only be issued by the Office
of the Registrar General for the Province of Ontario and
the forms are also available from the Clerk's Office.
